In addition to the above Service Activation Fee, User shall pay to Bay the additional monthly service fee
payable on the same terms as the service fee in the Agreement in accordance with all of the terms and
provisions of the Agreement.
Additional Monthly Service Fee $
All of the terms, covenants and conditions of the Agreement by and between Bay and User shall remain in full
force and effect, except as expressly modified by this Addendum. User acknowledges and agrees that this
Addendum and the equipment and/or services performed hereunder are subject to and provided pursuant to the
Agreement and particularly those sections which set forth Bay's maximum liability and right to indemnification
in the event of any loss or damage to User or anyone else
