In accordance with your request, we are pleased to submit the following proposal:
Scone of Work: To perform CCTV inspections of approximately 12,172 _F of 8'— 12' diameter
anitary sewer lines located at various locations in the District, as identified in the District's fax
dated 12'26/00. Inspections will be made in as-!s condition Inspections will be performed
n;th ciao- CCTV inspection equipment and recorded on stancard VHS equipment. Tapes and
WinCan og sheets will be submitted upon completion.
National Plant_Services Will Furnish:P CCTV inspection uni- and a uti.ity truck w:th a two i2)
person crew to perform tis work.
Costa Mesa Sanitary District Will Furnish. Complete access, openings and rights-of-way to
Me work areas; any local permits or licenses that may be requi>ec; any Mugging or by-pass
pumping that may be required;
Price. The following scnedule of rates and charges will apply to the performance of this work.
CCTV Inspections @ 5.49/LF
2 Sewer Cleaning _ 45.00/Crew Hour
3 Daly Report to Job Charges @ 360.00/Day
