Project 160 - Change Order #2 - Sully-Miller - 1999-05-05 Costa Mesa'Sanitary District CONTRACT CHANGE ORDER NO 2 PROJECT #160 Fairview Park Sewer TO Sully-Miller Contracting Co Contractor You are hereby directed to make the herein described changes from the plans and specifications, or do the following work not included in the plans and specifications on the contract. Note: This change order is not effective until approved pursuant to adopted District policy Change Requested by Sully-Miller Contracting Co e - Description of work to be done, estimate of quantities and prices to be paid. Segregate between additional work at contract price, agreed price and force account. Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. During trenching for the construction of new sewer line in Canyon Drive contractor's operation was slowed by approximately two hours per day due to an existing abandoned 4" water line that had to be removed from the trench prior to installation of the sewer The water line jogged from side to side in the trench in a random alignment and necessitated the contractor making a wider sewer trench Delays and- costs are further outlined in attached request for change order and associated costs which have been verified by District staff Net Increase © Decrease H in Contract $ 8,205.86 By reason of this order the time of completion will be adjusted as follows. Rob Hamers Dear Rob, There have been a couple of problems encountered on our project: Sully-Miller encountered an unforeseen water line, which ran the length of our sewer installation on Canyon Drive, this line was not shown on the plans. We are estimating that the line interference cost Sully-Miller 2 hours each day of lost crew time in the street portion of Canyon Drive, which was 500 feet. The lost time along with the disposal costs for this line is currently being calculated by Sully-Miller we feel the district should be responsible for these costs. The last item is in regards to the re-alignment of line C, which has been moved. Due to this movement Sully-Miller has had to repair additional sprinklers which we estimate we have costs of$500.00 to repair. Sully-Miller is diligently calculating the additional costs associated with these problems and will foreword these costs to you as soon as possible. Thank you for your cooperation.
