Contract - Curbside - 2011-06-23 ADDENDUM NO.3
This Addendum to Agreement Is made and effective as of 2011, between
the Cost Mesa Sanitary District,a sanitary district("District"),and W Curbside, LLC,a Delaware limited
liability company("Contractor').
RECITALS
WHEREAS, the District and Contractor entered Into an agreement dated October 12, 2005,
which provides for Contractor to perform the tasks described and set forth in the Door-to-Door
Household Hazardous Waste(HHW)Operational Plan,Exhibit A to the Agreement;and
WHEREAS,the parties amended the Agreement by Addendum No.1 to provide battery recycling
services, Exhibit B; and
WHEREAS,the parties amended the Agreement by Addendum No. 2 to extend the Agreement
dated October 12,2005, including Addendum No. 1,for an additional term and desire to change certain
pricing contained therein,Exhibit C,and
WHEREAS, the parties do hereby desire to extend the Agreement dated October 12, 2005,
including Addendum No. 1 and Addendum No.2 for an additional term
NOW,THEREFORE,the parties hereto do hereby agree as follows:
1. That the Agreement dated October 12, 2005, as amended to Include the battery
collection services set forth In Addendum No. 1,and to include the change in certain pricing set forth in
Addendum No.2,is hereby extended until June 30,2012.
2. That the collection locations shall be as mutually agreed by the parties In writing.
3. That, except as specifically amended above,the terms and conditions of the Agreement
and Addendum No.1 and Addendum No.2 shall remain in effect.
IN WITNESS WHEREOF the parties hereto have caused this Agreement to be executed this day
and year first above written.
