agrees to continue providing Employee Assistance Program services to the employees and family
members of Costa Mesa Sanitary District located at 628 West 19th Street, Costa Mesa, California
92627 the extended period will be from 7/1/2006 through 6130/2008. Inc annual contract amount for
services will he $24.00 per employee per year payable at $2.00 per employee per month in act'ance
based on an employee population of 7 This employee count will he utilized throughout the
contract year unless an increase or decrease of 10% or more occurs. adjustments to billing will occur the

The following represents a change to the original contract. The CONTRACTOR has developed a series
of professional promotional material. This contract will include 110"„ of the contracted employee
count of brochures and cards. All Shipping and handling expenses associated with the promotional
material will he billed to the COMPAN\ Any other materials, which the COMP&N1 may distrihute.
