We agree to make a minimum of ( 4 ) inspections per year at which time we shall perform the services listed below under "Definition of
Maintenance".

Service Technician shall inform customer of his findings, work performed, and recommendations pertaining to listed equipment. It is
mutually understood that the agreement price does not include the furnishing of replacement parts, refrigerant, or belts unless noted below.
DEFINITION OF MAINTENANCE
Maintenance is defined as the inspection, cleaning, and lubrication of listed equipment in order to maintain it in proper and efficient
working order. To achieve this, the Service Technician, at the above -prescribed intervals shall perform the following services he deems necessary
and applicable to the listed equipment.
Check refrigerant level
Chemical coil cleaning (extra)
Inspect equipment for refrigerant leaks
Inspect motors, blowers, and housings
Check and adjust fan belts
Test and calibrate safety controls
Lubricate all moving parts
Clean and adjust pilots and burners
Check condensate drains
Inspect flues and fireboxes
Check amperage of motors

Inspect condenser coils
Check thermostat operation
Check overall operation and performance
Priority shall be given to agreement holders over other service calls in order to expedite emergency service. We shall take all necessary precautions to
avoid injury to persons and damage to property.
This agreement is subject to revision on each effective anniversary date, and may be canceled by either party upon receipt of thirty (30)
days written notice. It is understood that this proposal sets forth our entire agreement and shall become a contract when accepted by the customer
and approved by us. It is further understood that this agreement is for Preventive Maintenance only.
This agreement shall automatically renew on each anniversary date subject to cost of inflation.
Maintenance shall be provided for a sum of $646.00 per inspection payable after service is performed._
