The Costa Mesa Sanitary District (District or CMSD) has adopted the Uniform Public Construction Cost Accounting Act (UPCCAA) and this project falls in the Informal Bidding range in accordance with bid limits adopted by the District. 2. The request for proposal (RFP), bid form, and sample contract may be obtained for a cost from Santa Ana Blueprint Reprographics (SABP), 2372 Morse Avenue, Irvine, CA 92614, (949) 756-1001 phone; workorder@sabp.com email. 3. Enter company information on the bid form and enter the individual bid amounts for each bid item and the total bid amount (number and words). 4. Return bids by email to both Rob Hamers, District Engineer, rhamers@robhamers.com 714- 293-2727, and Steve Cano, Wastewater Maintenance Superintendent, scano@cmsdca.gov. 909-910-5998. 5. Mandatory Pre-Bid Meeting • There will be a mandatory pre-bid meeting at 12 p.m. (Noon) on Friday, June 7, 2019. • The meeting will be held at the District Engineer’s office at 234 East 17th Street, Suite 205 in Costa Mesa, CA. Bid Requirements • Prevailing wages must be paid. • Contractors and subcontractors must be registered with DIR. • A or C42 license required. Bid Requirements (Continued) Protecting our community’s health and the environment by providing solid waste and sewer collection services. www.cmsdca.gov #324 Request For Proposals (RFP) Page 2 of 2 • Contracts over $25,000 require a labor and material bond and a faithful performance bond, both in an amount equal to 100% of the contract amount. No bidder’s bond is required. • After Notice to Proceed is issued, contractor shall begin work and complete within 30 working days. • Successful bidder will be required to enter into a comprehensive contract with District. (See attached sample contract.) • District may request references after bids are received. Special Provisions • Contractor shall furnish all labor, materials, and equipment for the rehabilitation of the ten (10) existing brick manholes with cured-in-place composite liner system per attached technical specifications. The liner system shall be Triplex Liner System, Alternative Liner System, or approved equal. • Contractor to provide engineered traffic control plans to be approved by the city of Costa Mesa and all other incidental items required for a turnkey installation. • Refer to bid form (attached).
