This had reference to report of completion of plans and specifications under General Service Administration Project Calif, 4 -P -1016. Engineer Bulot submitted completed plans and specifications under above Project and Advance for approval and acceptance by the Board of Directors. Whereupon a Resolution, copy attached and made a part of these minutes, upon motion by Lord seconded by Nelson, was adopted by the following roll call vote. An original and three copies of the form should-be submitted together with one set of completed drawings and specifications, and a certified copy of the resolution of the applicant's governing:body approving the completed drawings and'specifications, to the appropriate field office of Community Facilities Service, CAcac.a P =-s::Adrninistra-tim It is important that each item on GSA Form 9 be answered clearly and accurately; if space on the form is not adequate to permit a complete answer, a separate sheet containing the - additional or clarifying information properly referenced should be securely attached. Instructions governing the preparation of GSA Form 9 by applicants follow: Item 1. —Legal corporate name of the applicant and the official address. The name of the applicant should be in exact agreement with that contained in the offer to make an advance by the Government. Item 2. —The name, title, and address of the representative of the applicant who is authorized at the time of completion of plan preparation to act for the applicant in all matters pertaining to the approved advance. Item 3. —The name, address, and State license number of the architector engineer immed:iately responsible for the plan preparation at the time of completion. In the case of an architectural or engineering firm, the full :name and address of the firm should be shown. If plan preparation was performed by the applicant's forces, thename should be that of the individual who was responsible for the actual supervision of the plan preparation. Item 4. —A final complete but concise description of the proposed public work in accordance with completed drawings and specifications. With each completion report shall be submitted a set of the final drawings, specifications, and basic design data bearing approval of any State agency or department whose approval is required before construction can be started. The final drawings and specifications, and other pertinent documents, shall bear the seal of the grchitect or engineer where required. Item 5.— Location of proposed public work. It may be necessary to show the approximate location on a small scale map to be attached. Item 6. —The final estimated cost of the proposed public work based on completed plans segregated by type of expenditure. On an attach- ment to each copy of GSA Form 9, the subitem "Construction" shall be broken down by the various types of work and other principal classifications. For example, on building projects the break -down will include cost of excavation, masonry (brick" and concrete), struc- tural steel, roofing, electrical work, plumbing, heating, ventilating, or air - conditioning, and the work of each of the accessory trades, such as plastering, floor covering, and painting; on sewer projects the break -down will include the cost of pipe in place at the various depths of trench, and manholes shall be priced separately with depths. given; on water projects where the depth of trench required is uniform the estimated cost shall be broken down into the various pipe sizes, with valves, fire hydrants, booster stations -or other special features shown separately; on sewage treatment plants and water treatment plants the estimated cost should be broken down in a manner similar to that required for building construction with any built -in special features shown as separate elements of the estimate. The estimate of construction cost of other types of projects shall follow the pattern above outlined for buildings, sewer, and water projects. The figures given for the estimate of cost under Item 6 may vary from the original estimates submitted with the application, since costs may be more accurately determined after completion of plan preparation. Item 7. —Show the final cost of plan preparation segregated between those costs not chargeable against the advance and those costs charge- able against the advance, with a further division between work by contract and by applicant's forces. In the case of plan preparation costs by. the applicant's own forces being charged against the advance, an itemized break -down of the cost of the work showing number of persons employed, time engaged, and. rates of pay should be submitted as an attachment. The total cost of plan preparation (Item 7 (c) 3) must be in agreement with the figure therefor shown in Item No. 6, "Plan Preparation." Item 8. —State whether or not the completed drawings and specifications prepared by the applicant's architect or engineer have been offi- cially approved by resolution of the applicant's governing body. A certified copy o. the approving resolution or ordinance must be attached to the GSA Form 9. Payment of the balance of the advance will not be made unless such approval is received by the appro- priate field office of Community Facilities Service. Item 9. —State whether or not approval of plan preparation by any State agency or department is required before construction can be started. If such approval is required by law, final payment will be deferred until such approval is obtained. Item 10" Give date when the applicant intends to start construction of the proposed public work. If no date has been specifically fixed to initiate the construction, give your best estimate as to when construction will start. Item 11.— Indicate the present status of the site for the proposed public work. In the event the site has been selected, placed under option, or acquired, a check mark should be shown in the appropriate space. Item 12. —State whether or not the applicant will have the financial ability to construct the public work at the estimated total project cost and on the date fixed 'or' stated in Item No. 10. Attention is directed to the "Information for Applicants," transmitted with the applica- tion wherein it is stated that it is the "responsibility of the applicant to insure the development of a set of plans and specifications for the proposed public work which are in keeping with its financial ability." Item 13.— Statement of the contemplated sources of funds and amounts from each source to finance the construction of the specific public work for which plan preparation is being reported. The amounts shown and the sources may vary from statements made in the appli- cation for an advance for plan preparation, since the project financing may have advanced to a more definite stage than originally reported. The total on the last line of Item No. 13 should represent the grand total of all amounts reported under subitems "A" through "E" and should agree with the total estimated cost of theproposed public work reported under Item No. 6. . Item 14. —Space is provided for general comments. In the event the space is insufficient an additional sheet may be attached. Page 3 of this form is a suggested form of resolution of approval of the completed plans by the governing body of the applicant, and also a suggested form of certificate of recording officer. If these forms are usable by the applicant, the words "Suggested Form of" may be stricken at the top of each form and properly filled out by the authorized official of the applicant. �J ��� U. S.
