Complete INCIDENT REPORT form on MembersPlus Online (www.sdrma.org) and provide:
A complete description of the incident;
Any documentation
your agency has related to their investigation of the incident;
Police and/or fire reports, if available; and
Photographs, if available

When an injury first occurs, contact Company Nurse to report the incident and to see if immediate
medical care is warranted.
If the employee chooses to seek medical care, they will be directed to the Occupational Medical facility your agency has selected through the Medical Provider
Network, WellComp.
