In accordance with your request, we are pleased to submft the following proposal:
Eoaeo oFWak:To perform CCTV inspections of approximately 5,746 LF of 8'diameter sanitary sewer
tines located in Distriots Area 11 as Identified by the District's map and line segment Gat. Inspections will
be made in Inspections will be performed with color CCTV inspection equipment and
recorded on standard VHS equipment. CD/a and/or Tapes and WinCan log sheets will be submitted
upon completion.
National Plant Services Will Furnish:ACCTV Inspection unit and a utility truck wish a two(2)person
crew to perform this work.
Costa Mess Sanitery Aistriet WilLEainish_Complete access,openings and rights-of-way to the worts
areas;any local permits or licenses that may be required;any plugging or by-pass pumping that may be
required;any and all cleaning that may be required;
aim The following schedule of rates and charges will apply to the performance of this work.

2. Standby lima n$160.00/Hr
3. Optional to the District: CCTV Inspections c 3160.00/Hr ST
4. Travel for Optional Pricing i g Pay Report to Job Charge Q$100.00/Day
'Note:The CCTV per4Oot price is higher than normal due to the layout of the project and the increased
number of setups that wiiV be required. Due to the small size of the project and anticipated reduced
productivity,the per-fool price is higher than previous quotes to the District.
Termer Attached Terms and Conditions are incorporated into this proposal.

Customer's responsibility. if Contractor cannot continue Its work due to drtumstance caused or allowed by Customer and of which
Contracmrwas net apprised prior to starting the work,an hourly fee will be charged.
Qalnaae llos: Under no circumstances will the Contractor be responsible for indirect,mmIdental or consequential damages. The
Contractor else is not responsible for the rendering of or failure to render arthltecture,engineering Or Surveying professional services.

basement sewers)where the primary Cause of the daim or damage Is preexisting condition including faulty, inadequate or defective
design,constnraton,maintenance or repair of property or contamination of the subsurface where the condition anted prior to the start
of the Contractor's work. Customer is responsible for loss of sandee equipment causal by the pre-exleting conditions at the Job site.

The Cusmnter will be responsible for any additional costs or claims associated wee tae treatment, storage, disposal of the removed

Jbytmatfkglgni The Customer and the Contractor will eat* indemnify the other In proportion to relative fault for honesty, loss and
expense Incurred by the other party resulting from a negligent act or omission in performance of wade under this Agreement. The
Customer also will Indemnify Contractor for liability, loss and expense resulting from Contractors services if the Contractor Is acting at
the direction or Instruction of the Customer,or where the primary cause of any damages is due to information provided by the Customer.
Caelle_exeliev Regular Terms are Net 30 Oats. The Company may charge Interest at the rate of 1.1/2ts per month on all invoices
Outstanding 60 days past Invoice date.
