Contract - National Plant - 2010-01-28 ADDENDUM NO. 2
TO
AGREEMENT FOR SEWER LINE MAINTENANCE AND REPAIR
This Addendum to Agreement (AGREEMENT FOR SEWER LINE MAINTENANCE AND
REPAIR) is made and effective as of 28th day of January 2010 between the COSTA MESA
SANITARY DISTRICT a sanitary district ( 'District") and NATIONAL PLANT SERVICES,
INC (a California corporation) ("Contractor') with reference to the following facts:
RECITALS
WHEREAS, the District has over 1200.000 linear feet of sewer mains that must be
maintained by cleaning according to industry standards, and
WHEREAS, said industry standards suggest that lines be cleaned once every three years;
and
WHEREAS, District previously had contracted for sewer line cleaning and maintenance
with the City of Costa Mesa, and
WHEREAS, District terminated that relationship in 2006 and has determined to contract
with a private contractor for said services; and
WHEREAS, Contractor was awarded a contract on February 17 2006, for line cleaning
and cleaned the District lines and provided CCTV inspection and grading pursuant to National
Association of Sewer Service Companies (NASSCO) Pipeline Assessment and Certification
Program (PACP) standards and completed said work pursuant to Project 175 and
WHEREAS, pursuant to Addendum No 1 Contractor added Drainage Basin
Identification Numbers for the District-prepared drainage basins shown on maps, with line
diameters lengths and manholes shown and inputted into the WinCan database for use in future
maintenance cleaning, and further provided for a two-pass cleaning of over 236,000 linear feet of
sewer line; and
WHEREAS, the District does hereby desire to further contract with Contractor to clean
additional areas of District's system to keep District within industry standards for maintenance
and to amend the contract dated February 17 2006, and
NOW THEREFORE, the parties hereto agree as follows:
1 Contractor shall schedule and perform additional sewer line cleaning with a two-
pass technique, all as provided for in the Agreement dated February 17 2006, unless otherwise
provided in this Addendum.
2. Contractor shall add Drainage Basin Identification Numbers for the District
prepared wastewater drainage basins (maps (atlas), line diameters, line lengths (LF) and manhole
data) into the Contractors WinCan database for use in the maintenance cleaning project and
future scheduling as specified in Exhibit A at a cost of$3,125.00.
3 The linear footage to be cleaned shall be 400,000 linear feet, all as more
specifically provided for in Exhibit A to this Addendum, equating to the areas and footage of
District main line cleaned during time period July 1 2006 and June 30, 2007
4 The price for said work shall be $ 35 per linear foot for an estimated cost of
$140,000
5 All work shall be performed using two combination cleaning trucks and three
crewmen between two vehicles (2 operators and 1 floater) to clean an estimated 5.000 linear feet
per vehicle per day for a total of 10,000 LF per day
6. Contractor shall seal manholes District identifies using silicone sealant and cork. After
cleaning, the manhole cover will be placed in the ring. Silicone sealant will be applied into the space
between the ring and cover and the manhole cover will be installed into the ring; a stopper/cork/plug will
be placed into each identified vent and pick hole (leave one vent hole or pick hole open per manhole
cover).
7 Contractor shall perform and provide sample CCTV verification of the
maintenance line cleaning at a rate of 1% of the cleaned lines (an estimated 4,000LF of CCTV).
8. Contractor shall deliver the following documents:
a. A District Atlas Map (District provided) with all the District sewer line
segments cleaned under this agreement highlighted in yellow with the
corresponding date of cleaning.
b. A dated spreadsheet of Manl-Iole—ManHole sewer line segments cleaned
mirroring the CCTV database format delivered under the contract awarded
February 17 2006.
9 Total Cost for this Addendum No 2 shall not exceed $150,000
10. Contractor shall complete the maintenance line cleaning by June 30. 2010.
I I Except as modified herein, all terms and conditions of the Agreement for Sewer
Line Maintenance and Repair dated February 17 2006, shall remain in effect and apply to this
Addendum. Current insurance certificates shall be maintained on file with the District
throughout the course of this work.
IN WITNESS WHEREOF the parties hereto have caused this Addendum to be executed the

Re: District Sewer Maintenance Services 2009-2010
In accordance with your request,we are pleased to submit the following proposal:
Scope of Work: To perform two-pass maintenance cleaning of approximately 400,000 LF of 8' —12"diameter
sanitary sewers located in the Costa Mesa Sanitary District. Sewers to be cleaned are sewers that were cleaned and
inspected between July 1 2006 and June 30, 2007 During the course of this work, National Plant Services will seal
manholes identified by the District. On a monthly basis, 1%of sewers cleaned will be inspected.
Log sheets of cleaning activities and mpeg video files will be provided.
Work Plan
1 NPS will plan out the work based on the CCTV inspection dates A Cleaning Team consisting of two(2)
jet/vac units with a three(3)man crew will perform this work in a methodical, planned, basin-by-basin
progression. NPS will plan this work using District Sewer Maps(and Basin Maps, when available)and the
WinCan data base.
2. NPS will seal manholes identified by CMSD using silicone sealant and cork After cleaning,the manhole
cover will be placed. Silicone sealant will be applied into the space between the ring and cover a cork will
be placed into each pick hole
National Plant Services Will Furnish: All supervision, equipment and personnel necessary to clean and inspect
sewers as outlined in the Scope of Work, cleaning logs and inspection video files;
CMSD Will Furnish: Complete access, openings and rights-of-way to the work areas, any and all permits that may
be required, an unlimited water supply from the nearest available hydrant with roving water meter;any additional
traffic control that may be required in excess of NPS'truck-mounted arrowboards, cones and signs; disposal of all
debris collected at OCSD' facilities;
Price: The following schedule of rates and charges will apply to the performance of this work:
1 Maintenance Cleaning @$.35/LF
Estimated Cost 400,000 LF @ $.35/LF=$140 000.00
Terms: National Plant Services'Terms and Conditions are attached and incorporated into this proposal.
