NOTICE: Conditions arising that would support a request for payment for additional
work or time extension must be brought to the District’s attention within ten (10) days,
and a Request for Change Order for additional work/time extensions must be filed within
fifteen (15) days. THIS FORM MUST BE USED. (See Contract Section 5.)
Circumstances supporting payment for additional work/time:
1. Describe why payment should be granted. Attach any more detailed discussion,
with backup documents as an exhibit. (Remember: damages have been
contractually limited on this job. See Section 28 of the Contract.)
The District completed the arc-flash review and study work that identified
urgent work that must be conducted because the electrical deficiencies
create a hazard to the station and District staff. The added work includes
design for:
A. Irvine Station:
Installation of a main electrical breaker, local disconnect for the
generator set, and replacement interlock for the generator set and
portable generator plug.
B. Harbor Station:
Replacement of the portable pump transfer switch and replacement of
the undersize main wire/conductors between the main breaker and buss
bar/pump breakers.
Summary
The total requested change order cost is $7,422. This change order
request amounts to an 18.7% increase, based on the $39,710 contract
amount. The proposed change order price is reasonable, based on District
staff soliciting bids from two other firms, Linkture Electrical, at $9,830, and
a “No Bid” from Spec Services Electrical Engineering Services. The District
Engineer also called Carollo Engineers and Black and Veatch, neither
would respond to this small engineering service and declined to provide an
email.

2. Describe the delay and why a time extension is appropriate. Provide any backup
documents as may be necessary to support the request. You must demonstrate
how the critical path was impacted.
A hundred (100) workday extension is being granted to account for delays
caused by the District and to provide time for the Consultant to prepare the
project design.
