STREET RESURFACING REQUIREMENTS

NEBRASKA PLACE IS UNDER A "NO OPEN CUT" MORATORIUM UNTIL 8/17/2006. COMPLY
WITH THE FOLLOWING MILL & OVERLAY REQUIREMENTS IF THE STREET IS CUT OR
DAMAGED.
SPECIAL NOTE No. 2
MINNESOTA AVENUE IS SCHEDULED TO BE ASPHALT OVERLAID IN 2004. O.C.S.D. SHALL
COORDINATE IT'S WORK WITH THE CITY INSPECTOR. O.C.S.D. SHALL COMPLETE IT'S WORK PRIOR
TO THE PLACEMENT OF THE ASPHALT OVERLAY OR COMPLY WITH THE FOLLOWING MILL &
OVERLAY REQUIREMENTS IF THE STREET IS CUT OR DAMAGED.
MILL & OVERLAY REQUIREMENTS
1. The contractor shall restore the roadway surface within the work limits by milling
and overlaying, minimum of 2 inches, the entire lone(s) where the trench is
located.
A. For trenches perpendicular to traffic lanes: mill and overlay shall extend 50
feet on both sides of the trench and will include the entire travel lanes.
B. For trenches parallel to traffic lanes and for small excavations: mill and
overlay the entire width of the traffic lane for the full length of the trench,
but not less than 100'.
2. Restoration of the roadway shall be continuous and no joints will be allowed. All
roadways within the work limits will be restored in one operation. No piecemeal
milling and overlay work will be permitted. The contractor shall not proceed with
the milling and overlaying operations until direction is given by the City Engineer.
3. Any existing improvements damaged by the Contractor shall be replaced as
directed by the City Engineer.
4. Asphalt concrete shall conform to section 302 -5 of the Standard Specifications
for Public Works Construction. Asphalt shall be Type III —C3 —AR -4000.
GENERAL NOTES CITY OF COSTA MESA
1 ALL WORK AND MATERIALS WITHIN THE PUBLIC RIGHT —OF —WAY SHALL CONFORM TO THE CITY OF COSTA MESA STANDARDS
AND SHALL MEET THE APPROVAL OF THE CITY ENGINEER.
2 ALL WORK SHALL BE PERFORMED AND ALL MATERIALS SHALL BE IN ACCORDANCE WITH CITY OF COSTA MESA STANDARD
DRAWINGS AND THE STANDARD SPECIFICATIONS FOR PUBLIC WORKS CONSTRUCTION, GREEN BOOK) CURRENT EDITION.
3. CONTRACTOR SHALL FURNISH AND INSTALL ALL SIGN, LIGHTS, BARRICADES, FLASHING ARROW BOARD AND ANY OTHER
TRAFFIC CONTROL OF WARNING DEVICES, INCLUDING FLAGMENT, AS MAY BE REQUIRED BY THE CITY ENGINEER. ALL ITEMS
MENTIONED SHALL BE IN CONFORMANCE WITH THE CURRENT REQUIREMENTS AS SPECIFIED IN THE LATEST EDITION OF THE
"CONSTRUCTION SAFETY ORDERS," AND W.A.T.C.H. FAILURE TO SO WILL BE CITED UNDER APPLICABLE SECTION OF THE C.V.0
AND CORRECTIVE COSTS TO THE CITY WILL BE CHARGED.
4 OCSD AND CONTRACTOR SHALL OBTAIN PUBLIC SERVICES DEPARTMENT ENCROACHMENT PERMITS PRIOR TO THE START OF
AND WORK WITHIN CITY PARKS OR OFF —SITE CONSTRUCTION IN PUBLIC RIGHT —OF —WAY AND REQUEST INSPECTION 24 HOURS
IN ADVANCE OF PERFORMING ANY WORK. TELEPHONE (714) 754 -5025. THE CONTRACTOR SHALL MAINTAIN A COPY OF
PERMITS AND SIGNED PLANS ON THE JOB SITE.
5 ALL EXISTING DRAINAGE COURSES ON THE PROJECT MUST CONTINUE TO FUNCTION, ESPECIALLY DURING STORM CONDITIONS,
AND APPROVED PROTECTIVE MEASURES AND TEMPORARY DRAINAGE PROVISIONS MUST BE USED TO PROTECT ADJOINING
PROPERTIES DURING THE GRADING PROJECT. IN ALL CASES, THE CONTRACTOR SHALL BE HELD LIABLE FOR ANY DAMAGE
DUE TO OBSTRUCTION OF NATURAL DRAINAGE PATTERNS. IF THE FOREGOING CANNOT BE COMPLIED WITH, A FULLY
EXECUTED, NOTARIZED, AND RECORDED AGREEMENT BETWEEN THE AFFECTED PARTIES WILL BE REQUIRED. ALL HAUL ROUTES
MUST BE APPROVED BY THE MANAGER OF TRANSPORTATION SERVICES.
6 THE PROJECT SITE SHALL BE MAINTAINED IN A "WET —DOWN" CONDITION TO PREVENT EXCESSIVE DUST. SPILLAGE AND DUST
SHALL BE REMOVED FROM THE PUBLIC RIGHT —OF —WAY BY SWEEPING OR SPRINKLING; HOWEVER, WATERING, THAT RESULTS IN
MUD ON PUBLIC STREETS IS NOT PERMITTED AS A SUBSTITUTE FOR OTHER CLEANING METHODS.
7 ALL UTILITY TRENCH BACKFILL, MATERIAL, AND COMPACTION SHALL CONFORM TO CCM STANDARD NO. 813. BORE UNDER
EXISTING IMPROVEMENTS — TUNNELING IS NOT ALLOWED. NO OPEN EXCAVATIONS WILL BE ALLOWED OVERNIGHT — BACKFILL OR
PLATE.
8 ALL PCC AND AC TO BE JOINED SHALL BE SAWCUT IN A CLEAN, STRAIGHT LINE AS APPROVED BY THE CITY ENGINEER.
9 ALL OBSTRUCTIONS, WITHIN THE PUBLIC SIDEWALK SUCH AS POWER HOLES, STREET LIGHTS, FIRE HYDRANTS UTILITY
PEDESTALS, MAILBOXES, AND STREET SIGNS SHALL HAVE MINIMUM OF 4' OF CLEAR SIDEWALK PLACED BEHIND THEM.
10 NO LANE CLOSURES WILL BE ALLOWED BETWEEN THANKSGIVING DAY AND NEW YEARS DAY.
11 "ASBUILT" DRAWINGS SHALL BE SUBMITTED TO THE CITY ENGINEERING DIVISION UPON COMPLETION OF IMPROVEMENTS AS
SHOWN ON THE APPROVED PLANS.
12. REPLACE / REINSTALL NEW STRIPING, PAVEMENT LEGENDS AND STOP BARS DAMAGED BY CONSTRUCTION.
13. ALL CONSTRUCTION STAKING SHALL BE DONE BY A LICENSED ENGINEER AND CERTIFIED IN WRITING AS TO LINE AND
GRADE PER APPROVED PLAN AND SUBMITTED TO THE CITY ENGINEER. OCSD WILL PROVIDE STAKING, SEE
SPECIFICATIONS, DIVISION 01, SECTION 36.
14. WORK, ONCE BEGAN, SHALL BE PROSECUTED TO COMPLETION WITHOUT DELAY SO AS TO PROVIDE MINIMUM
INCONVENIENCE TO ADJACENT PROPERTY OWNERS AND TRAVELING PUBLIC. MAINTAIN ACCESS TO ALL RESIDENTIAL
PROPERTIES AS NECESSARY.
15. ALL UNDERGROUND WORK SHALL BE COMPLETE PRIOR TO ANY SURFACE WORK IN THE PUBLIC RIGHT —OF —WAY.
